Humans, yet again!

Cases considered -

1) Lake Peigneur Salt Mine Disaster

2) San Fransisco Sewer Collapse

3) Hyatt Regency Walkway Collapse - 1981

This comes as no surprise, the films shown on this day showed us how, in 2 out of the 3 cases humans were somewhat directly responsible for the loss of so many lives. Starting with the Hyatt Regency walkway collapse that occured on July 17, 1981, where the whole reason for the collapse was overlooking some major calculations while approving the project, simply based on the fact that everyone assumed that someone else had already took care of that was a stellar example of the consequences our laziness can lead too. The Lake Peigneur salt lake disaster is an example where overestimating calculations caused a lot of structural damage. Thankfully there wasn't any loss of life, but the company was made to pay $32 million for the damage they caused to the lake and the surrounding areas.
