Project Format:  This brochure will be a teaching tool about hypertension in the community . It will definitely improve  their health literacy about this very common condition, discuss the importance of compliance to their medication to prevent late complication  of hypertension and also lifestyle modification such as diet , exercise, smoking and alcohol cessation, weight loss program.

Statement explaining how this course fits into the overall academic curriculum and objectives of this student:

I already took research writing , evidence based medicine and clinical pharmacology. This practicum will be an extension on what I have learned  so far. I am very excited to apply that  knowledge in a clinical/community setting for health promotion , education and disease prevention by increasing the health literacy for this community to have a better and healthier life.

I have recently completed a two week-course in Global health and Emergency preparedness at Cornell University  Medical  from March 3-16, 2012  that will help as far as organizing , planning and designing that health brochure to educate that community about hypertension.

At the end of the project I plan to achieve the following :

1. Improve patient education about hypertension for adults - which is the most common condition affecting this community I will provide them educational materials all in Creole  simple and easy to understand (second grade level health literacy )

2. Train teachers and community leaders to launch basic health maintenance program- more focused on prevention, education and compliance on hypertensive medications
